The certificate of membership of the Romanian College of Physicians provided for in paragraph (1) is issued on the basis of the following documents: a) application form; b) copy of the identity document; c) notarized copy of the graduation diploma from the faculty; d) copy of the official qualification title(s) in the medical profession or, as the case may be, the document attesting their recognition; e) proof of practicing the medical profession in the last 5 years, respectively a certificate from the workplace, if applicable, or proof of professional competence; f) criminal record or certificate of professional integrity issued by the member state of origin or provenance; g) A5 type health certificate, which must also include the psychiatrist’s endorsement; h) affidavit regarding the absence of any situation of incompatibility or unworthiness provided by law; i) copy of marriage certificate, if applicable; j) copy of the proof of payment of the registration fee.*
