Appendix description to the request*

Consent declaration regarding the processing of personal data*

Passport/identity card or ID in original and copy “certified as true to the original” (with the clear signature of the official receiving the application and the round stamp)*

Special power of attorney (for submitting the waiver request) authenticated by a notary or by the diplomatic mission of Romania. The representative will also submit a copy of their own identity document*

Certificate regarding debts to the state issued by the local taxes and duties department of the town hall from the last domicile in Romania*

Declaration on own responsibility, authenticated by a notary or by the diplomatic mission of Romania, regarding debts to natural or legal persons in Romania*

Civil status documents (birth certificate, marriage certificate, certificate of name/surname change or divorce), in notarized copies. If the birth/marriage/divorce took place abroad, the transcribed certificate must be submitted to the civil status office at the last domicile in Romania, in a notarized copy. If the documents have not been transcribed, notarized copies of them must be submitted, together with the legalized translation in Romanian, with apostille or superlegalization (as applicable).*

Promise of granting citizenship of the country of domicile, in a legalized copy with apostille/superlegalization (if applicable) and legalized translation in Romanian or Proof of obtaining citizenship of the country of domicile (naturalization certificate/proof certificate of obtaining foreign citizenship), in a legalized copy, with apostille/superlegalization (if applicable) and legalized translation in Romanian or passport issued by foreign authorities, in a legalized/"certified as original" copy with the clear signature of the official responsible for receiving the application and round stamp*

the minor's birth certificate, in a certified copy, transcribed, if applicable*

for the minor who has reached the age of 14, their notarized consent given in front of a notary or consul in the presence of a parent must also be submitted*

the parents’ own responsibility statement that the minor resides abroad*

ensuring the acquisition of another citizenship in which minors who will acquire the citizenship of the country of residence are mentioned or proof that they have another citizenship*

the authentic consent of the other parent, Romanian citizen (in the situation where only one of the parents requests the renunciation of Romanian citizenship)*

If the minor child has been entrusted by a court decision to the parent who has domicile abroad and who renounces citizenship, a certified copy of the court decision entrusting the minor, final and irrevocable, must be submitted.*
