Recent medical documents, recommendations from the specialist doctor, accompanied by documents issued by the competent authorities in the field of health social insurance certifying that the medical investigations, surgical interventions, drug treatment, medical devices or necessary therapies recommended by the specialist doctor are not covered by the health social insurance system or, if they are partially covered, the amount must be specified; copy of the document on the basis of which approval for treatment abroad was granted, supporting documents for the necessary expenses to be incurred, issued by the medical service provider who will carry out the surgical intervention/postoperative treatment, translated by an authorized translator, documents issued by the competent authorities in the field of health social insurance certifying that these are not covered by the health social insurance system. Supporting medical documents may also include, as appropriate: proforma invoices/fiscal receipts/receipts, certificates issued by sanitary authorities, letters, certificates or medical prescriptions, etc;*

Evidence regarding the covering of funeral expenses in original (expenses for food products are not reimbursed);*

Proforma invoices, receipts, fiscal receipts, estimates for assessing expenses with materials and labor required for executing certain works and/or projects, reports, resolutions, orders, or decisions issued by competent authorities in the field stating the existence of an emergency situation such as fires, floods, regarding the status of the property (for example, land registry excerpt), documents regarding the non-coverage or partial coverage of damages issued by insurance agents, copy of insurance policy/contract or, if the property is not insured, a declaration on one's own responsibility, etc.;*

Certificate issued by the County Agency for Employment for adult persons who do not have income from salaries, are not of retirement age, or do not have health problems, indicating whether they benefit from unemployment aid and whether they are registered as job seekers and that they have not unjustifiably refused any job;*

The rental contract registered with the National Agency for Fiscal Administration;*

Final court decision for eviction, notification or summons for eviction from the judicial executor and certificate proving that they have submitted a file for the granting of social housing;*
