In the case of incorrect completion due to the fault of the educational institution issuing the original diploma or in the case of presenting the original diploma damaged to such an extent that it can no longer serve its intended purpose, based on a written request from the applicant, the educational institution that issued the respective document shall issue a duplicate, prepared on the basis of documents held in the archive of the educational institution that attest to the applicant's academic status.
In the case of loss, destruction, or theft of the original diploma, the application for issuing the duplicate must be accompanied by the following documents, which are archived for a period of 100 years at the school inspectorate, with the “certified true copy” copies kept at the educational institution:
a) a notarized/consular declaration made by the holder of the document, including all necessary identification details, the circumstances under which the original document was lost, as well as other clarifications to confirm the disappearance of the diploma. The following statement must be included: "the original diploma was not left as pledge/guarantee at a state or private institution, either in the country or abroad, and was not knowingly transferred to another individual or legal entity";
b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;
c) a notarized copy of the birth/marriage certificate;
d) two color photographs, size 3 x 4 cm;
