The transfer note is issued by the college where the pharmacist is registered, based on a request.
Before the transfer note is issued, the requesting pharmacist is required to pay up to date the dues owed to the college from which they wish to leave. The transfer note will be handed, under signature, to the applicant. In the pharmacists' registration record (https://portal.tm-c.eu/LstFarma/ColegFarm) the college to which the pharmacist has been transferred will be mentioned. If a complaint has been filed or a disciplinary investigation has been initiated at the college from which the pharmacist is transferring, the complaint or the disciplinary file will be sent to the college to which the pharmacist is transferring.
