Details

In the event that the holder loses, in any way, the Certificate of Membership of the College of Pharmacists of Romania, a duplicate will be issued upon request. The duplicate is issued based on a request submitted to the College of Pharmacists where the pharmacist is registered as a member of the College of Pharmacists of Romania, as well as proof of publication of the loss in a nationally circulated newspaper. The duplicate will bear the series and number of the original certificate, followed by a number to certify/reflect its status as a duplicate, namely the number of copies issued.
