Details
The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is required to pay all dues owed to the college they wish to leave. The transfer note will be handed over, upon signature, to the applicant. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If, at the level of the college from which the pharmacist is transferring, a complaint has been filed against them or a disciplinary investigation has been initiated, the complaint or the disciplinary file will be forwarded to the college to which the pharmacist is transferring.
