The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is obliged to pay in full the membership fee owed to the college from which they wish to leave. The transfer note will be handed over to the applicant, with signature. In the pharmacists' registration record (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint has been filed against the pharmacist or disciplinary investigation has been initiated at the college from which the pharmacist is transferring, the complaint or the disciplinary file will be forwarded to the college to which the pharmacist is transferring.
