Details

In order to issue a duplicate of the Membership Certificate of the College of Pharmacists of Romania, the applicant will submit or send electronically to the Călărași College of Pharmacists the standard application and the complete documentation, according to the provisions of Decision no. 4/2004, amended and supplemented by Decision no. 2/24.11.2022, of the National Council of the College of Pharmacists of Romania.

All documents will be signed and will have the mention Certified true copy. The fee of 150 lei is to be paid into the account of the College of Pharmacists of Romania:
