The allowance is granted based on an application and a declaration on own responsibility, accompanied by supporting documents regarding the family's composition, its income, and, where applicable, regarding the attendance of school courses by the dependent children. Required documents for the file (in copy): – application and declaration on own responsibility; – identity documents of the applicants; – birth certificates of the children, their identity cards (where applicable); – marriage certificate or family record book; – final decision authorizing adoption, familial placement of the minor, according to the law; – document proving the applicant's quality as guardian or curator; – documents proving that a family member attends a form of education (school certificate for all school-age children in the family; exceptions are made for school-age children with severe or pronounced disabilities, for whom proof issued by the competent authorities must be provided, attesting that the respective child cannot be enrolled in any form of education provided by law); – documents showing inclusion in the category of persons with pronounced or severe disabilities or first or second degree invalidity, for dependent persons; – net income earned (the month prior to submitting the application) and the supporting documents are those mentioned in the application form (certificates regarding net salary, pension coupons, unemployment benefit, state allowance coupon, etc); In the case of consensual union (cohabitation), considered a beneficiary family, documents regarding family composition and income for the applicant's partner will also be presented.*
