Required documents for the file (in copy): – application form filled in and signed by the legal representative of the child; – original and copy of the child's birth certificate; – identity document of the legal representative; – documents proving the child's legal situation in relation to the legal representative and any other supporting documents, if applicable; – if the applicant chooses to receive the state allowance in a personal account, a bank statement and IBAN account from the bank are required.*

Required documents for the file (in copy): – standard application form – identity documents of the applicants; – child’s birth certificate; – as the case may be, marriage certificate or family record book; – certificate of classification of the child in a degree of disability, if applicable; – the request approved by the management of the unit for taking child-rearing leave; – proof issued by the employer or competent authorities regarding the income earned; – the decision to suspend the individual employment contract for a period of 1 year, 2 years, or 3 years in the case of a child with a disability; – in the case of persons who simultaneously earn taxable income from several sources, the leave and allowance for child-rearing is granted based on supporting documents proving suspension of at least one of the activities; – the tax record belonging to the applicant (if applicable); – if the applicant chooses to receive the allowance or incentive in a personal account, a bank statement is required, the IBAN account from the bank. If a person is already receiving the child-rearing allowance and gives birth to another child, a new file is to be made for the extension of the leave. If the person on child-rearing leave opts for the incentive payment, a new file is to be made and an application must be submitted for granting the incentive. Starting with March 1, 2012, at least one month of the total duration of the child-rearing leave is allocated to the person who has not requested this right. Therefore, regardless of the situation, the parent on child-rearing leave or receiving the insertion incentive, except in cases where the applicability of art. 11 of GEO no.111/2010 is not mandatory, will lose one month of entitlement, regardless if it is the allowance or the incentive, if the other parent does not access their own right to leave. The new regulation applies to children born after March 1, 2012, this being imposed based on the principle that the responsibility for caring for the child belongs to both parents.*
