Persons who do not receive another social benefit (e.g.: family support allowance or social assistance) fill in Annex no. 1 to the Government Decision - Annex no. 1*

Persons who benefit from family support allowance will complete Annex 2 to the Government Decision - Annex 1a*

- identity document of the applicant and all family members over 14 years old (B.I., C.I., C.I.P.); - birth certificate of children under 14 years old; - marriage certificate – if applicable; - divorce decree/notarial act - if applicable;*

- sale-purchase contract; - sale-purchase contract with maintenance clause or with lifelong habitation clause; - rental contract; - loan for use contract; - heir certificate/succession act; - other documents, as applicable.*

- pension vouchers (for all types of pension) / certificate from the Pension House from the month prior to submitting the application; - payment voucher for unemployment allowance and/or the monthly supplementary unemployment income; - state allowance, foster care or other allowance voucher, from the month prior to submitting the application; - disability allowance/complementary budget voucher or any other allowances granted according to the law – from the month prior to submitting the application; - income earned from lease, bank interest, sale of goods – if applicable; - self-declaration of the holders/members who do not earn income – if applicable. The copy will be certified handwritten for compliance with the original. - employment certificate for all family members, showing the net income obtained in the month prior to submitting the application, the value of meal vouchers or the statement that meal vouchers are not received, bonuses, food allowances, etc. – in original.*

- persons who own cars - copy of the car's registration book (showing the year of manufacture of the car); -bank statement The copy will be certified by hand regarding conformity with the original.*
