The request of the beneficiary or the owner, as the case may be, specifically mentioning the area, the investment to be made, Land Register number, Cadastral number and/or topographic number, signature/stamp (legal entities), accompanied by a copy of the identity document;*

In case of representation, notarial power of attorney, respectively delegation, general meeting decision of the associations, decision of the sole associate, decision of the representative of the associative form, as applicable, in original and copy, as well as a copy of the BI/CI of the authorized natural person or a copy of the passport for the authorized natural person with domicile abroad;*

Copy of the ownership document (for example; title deed, sale-purchase contract, donation contract) or another valid land holding document (for example; concession contract, lease, rental, superficies, etc) with notarized authentication of rental; (except for requests for works provided in art.7 paragraph 14 of Law no. 50/1991 regarding the authorization of the execution of construction works, republished);*

The owner’s consent for the implementation of the investment objective on the studied site, in case the land is owned by a person other than the owner; the administrator’s consent for the implementation of the investment objective on the studied site, in case the land belongs to the public/private domain of the state - in original and copy;*

Copy of the urban planning certificate (within its validity period), accompanied by the zoning plan and the site plan, as applicable;*

Payment can be made using the following methods: - cash at the central unit - bank transfer at the banking unit - treasury ANIF VASLUI ACCOUNT RO58TREZ656501701X006407*

The land register excerpt for information accompanied by the cadastral plan excerpt, issued by the office of cadastre and real estate publicity no more than 30 days before the date of submission of the documentation, as applicable (except in cases where the construction permit is requested for the realization and rehabilitation of improvement infrastructure in accordance with the provisions of art. 33, para. 2, letter a of Law 138/2004); (except for requests for works provided in art. 7, para. 14 of Law no. 50/1991 regarding the authorization of construction works, republished); (except for public utility investment works).*

Copy of the ANIF approval for drafting the ZUP (accompanied by the L.C.D. approving the ZUP), for verifying compliance with the conditions imposed when issuing the approval (if applicable);*

Where the situation requires it (when in the Urbanism Certificate, according to Law no. 133/2012, there is a mention of agricultural intravilan, or upon the request for a Detailed Urban Plan when there are IF works that may be affected), the beneficiary's file must include a Project with the technical solution for protecting IF works, signed and stamped by an ANIF authorized person, and also, according to art. 47^1 of Law no. 350/2001, republished, this must exist in the file.*

Certificate of registration and certificate of verification from the Trade Register or the document based on which the activity is carried out, in copy, in the case of the legal entity beneficiary;*
