Supporting documents showing that the holder has other persons in their care and the identity documents of these persons*

- certificate from the workplace stating the net monthly income earned in the last 12 months prior to the month in which the application is submitted (for all employed family members); - pension slip; - unemployment slip or certificate from AJOFM (as applicable); - certificate issued by the General Directorate of Public Finance of Bihor County regarding income earned as a result of performing a legally constituted income-generating activity (civil contracts, self-employed, copyright, dividends, shares or similar); - notarial declaration for adults who do not earn income.*

Notarial declaration, given by the applicant as well as by his/her first-degree family members who live and manage the household together, stating that: a. they do not own a dwelling b. they have not owned a dwelling which they have alienated after January 1st, 1990 c. they have not lost, in any way or due to their own fault, the right to a housing contract after January 1st, 1990 d. they have not benefited from state support in loans, subsidies, or other facilities for the construction of dwellings or other real estate investments e. they do not possess, as a tenant, another dwelling from the state housing fund*

a. Loan agreement, rental contract/convention or certificate issued by the Real Estate Heritage Directorate for persons accommodated in the “Dignitas” center or in accommodation centers/dormitories managed by the Real Estate Heritage Directorate. b. Proof of eviction from the dwelling restituted under Law no. 112/1995, Law no. 10/2001 or Law no. 501/2002, if applicable (e.g. eviction report drawn up by a bailiff or any other document proving that you have left the dwelling); c. Proof that the property in which the applicant and their family live has been restituted to the former owners.*

- For persons classified in degree 1 or 2 of disability/invalidity – the certificate of classification in degree of disability/invalidity; - Medical certificates regarding the health status for the applicant and their family members, and in the case where the applicant or a family member suffers from one of the diseases listed in Annex no. 2 of GEO no. 40/1999, which requires the allocation of an extra room, a medical certificate issued by the competent medical institution, signed by the specialist doctor and the head of that institution, must be presented.*

For war veterans and widows, beneficiaries of the provisions of the Law of Gratitude towards hero-martyrs and fighters who contributed to the victory of the Romanian Revolution of December 1989, as well as towards persons who sacrificed their lives or suffered as a result of the anti-communist workers' revolt in Brașov in November 1987 no. 341/2004, with its subsequent amendments and completions, and the provisions of Decree-Law no. 118/1990 regarding the granting of certain rights to persons persecuted for political reasons by the dictatorship established beginning March 6, 1945, as well as those deported abroad or taken as prisoners, republished, with subsequent amendments and completions, other entitled individuals or families – documents attesting the status of beneficiary of these legal provisions.*
