regarding the applicant's quality and their relationship with the child/children for whom the right is requested, for one of the persons who has adopted the child/to whom the child has been entrusted for adoption/ who has the child in foster care/emergency foster care, except for the professional maternal assistant who can benefit from these rights only for their own children, as well as the person who has been appointed as guardian. In the case of PFA (authorized natural person): - Certificate from the Health Insurance House (C.A.S) with the period during which the maternity allowance (pregnancy + postpartum) was paid or a declaration on own responsibility that the maternity allowance was not received; - Certificates from the Public Finance Administration - showing the income earned in the last 2 fiscal years prior to the child's birth or, as appropriate, proof of income calculated for the advance payment of income tax; - Tax imposition decisions from the Public Finance Administration for the last 2 fiscal years prior to the child's birth showing the start date of the activity, - Single Declaration, for the last 2 fiscal years prior to the child's birth date. - Suspension or termination of activity decision, declaration 070, from the Public Finance Administration. For students, university graduates, pupils, etc.: - Certificate issued by the educational institution, attesting the uninterrupted attendance of day courses up to the request date in the year prior to the child's birth date, or graduation diploma accompanied by the transcript or certificate issued by the higher education institution if the faculty was graduated in the year prior to the child's birth date if in one of the assimilated periods: - attending, without interruption, the day courses of pre-university education, including within the "Second Chance" program, or, as appropriate, university courses at the bachelor’s or master’s level, as well as post-university courses at the master’s level, organized according to the law, in the country or abroad, in a field recognized by the Ministry of National Education, except in cases where courses are interrupted for medical reasons; - holds doctoral status, under the provisions of National Education Law No. 1/2011, with subsequent amendments and completions; - is in the period between the completion of a form of pre-university education and the start, within the same calendar year, of another form of pre-university education, day courses, organized according to the law, attended without interruption; - is in the period between graduation from the day courses of pre-university education, organized according to the law, and starting university education, day courses, in the same calendar year; - is in the period between completion of a form of university education, day courses, with or without bachelor's exam or diploma exam, and the start, in the same calendar year, of another form of university education, day courses, organized according to the law, attended without interruption; - is in the period between completion of a form of university education, at the bachelor's or master's level, as well as post-university education at the master’s level, day courses, and the start, within the same calendar year, of another form of university education at the bachelor's or master's level, day courses, organized according to the law, attended without interruption; - is in the period between completion of a form of post-university education, day courses, and the start, within the same calendar year, of another form of post-university education, day courses, organized according to the law, attended without interruption; - is in the 60-day period from the completion of compulsory education or, as appropriate, from graduation of day courses in pre-university, university education at the bachelor's or master’s level, and post-university education at the master’s level, organized according to the law, with or without graduation exam, for the purpose of employment or, as appropriate, entering unemployment, calculated from the 1st day of the month following the completion of studies. - declaration on own responsibility that taxable income is not earned; In the case where the father requests the child care and upbringing allowance, a certificate from the mother's employer specifying the period in which the maternity allowance was paid, i.e., prenatal and postnatal leave, or a declaration on own responsibility that the maternity allowance was not received; - certificate issued by the educational institution, attesting uninterrupted attendance of day courses up to the request date in the year prior to the child's birth date, or a graduation diploma accompanied by the transcript or a certificate issued by the higher education institution if the faculty was graduated in the year prior to the child's birth date if in one of the assimilated periods.*
