Details
For obtaining the medical record (for applicants for a driving license and for vehicle drivers requesting the renewal of their driving license) and the medical certificate (to certify the state of health, for procurement, possession, carrying, and use of lethal or non-lethal weapons and ammunition subject to authorization), applicants must present themselves at the Outpatient Department (at the FILE, after making a prior appointment at the phone number 0770-320.609) with the following documents:
- Certificate from the family doctor, mentioning the medical history;
- Medical record completed in 2 copies (for applicants for a driving license and for vehicle drivers requesting the renewal of their driving license) or, as applicable, the medical certificate completed in a single copy (to certify the state of health, for procurement, possession, carrying, and use of lethal or non-lethal weapons and ammunition subject to authorization); (Annexes)
- Proof of payment for obtaining the medical record/medical certificate, according to internal regulations; (Payment is made at the cashier of the medical unit.)
- Identity card.
