Attention to applicants for social investigations for children, students, or young people with SEN (special educational needs)
In the case of individuals from the municipality who present themselves (initially/periodically, for re-evaluation) to the specialized commission within CJRAE Vâlcea, for obtaining the certificate of school or professional orientation, the documentation required for the preparation of the social investigation is as follows:
- identity document (child, parents - including the original)
- divorce decree / placement decision (family, foster care)
- court decision establishing guardianship/curatorship
- certificate of classification in degree of disability (if applicable)
- school and professional orientation certificate (if applicable)
- kindergarten/school certificate (if applicable)
- certificates regarding the net income of parents/legal representative
- tax certificate (for the parent/legal representative who does not earn income)
- proof of the establishment of protection measures (guardianship/curatorship/placement)
- the minor's medical documents (requested by the commission)
- proof documents regarding the therapies undergone by the child
- standard request (can be downloaded from the website)
