Issuance of medical documents for obtaining/changing the driver's license*
The procedure is addressed to all applicants who wish to undergo the medical examination for obtaining/extending the driver's license, within the Specialty Outpatient Clinic of the "Dr. Ștefan Odobleja" Military Emergency Clinical Hospital Craiova.
Reimbursement of the personal contribution for the release of medicines for the categories of persons provided for in Order M 110/2009 updated, from the territorial area assigned to the Dr. Ștefan Odobleja Craiova Emergency Military Clinical Hospital.*
The procedure applies to persons in the territorial allocation area of the Dr. Stefan Odobleja Craiova Military Emergency Clinical Hospital (SCUM Craiova) - persons from the counties of Dolj, Gorj, Mehedinți, and Olt - according to Order M110/2009 art. 4, letters f.), h), j).
Persons who do not find all the medicines at pharmacies contracted with SCUM Craiova can collect their prescriptions from any community pharmacy for a fee, requesting the following documents:
- fiscal invoice - original,
- fiscal receipt - original,
- simple prescription/CNAS type prescription (electronic) - "prescription" component and "release" component - original.
To recover the personal contribution paid for medicines prescribed on C.N.A.S. (electronic) or simple prescriptions, the personnel categories listed above shall prepare a report to the commander of SC.U.M. Craiova (according to the model attached to the procedure or the form posted on the hospital's website in the "Documents" section) accompanied by the following documents:
· fiscal invoice - original,
· fiscal receipt - original,
· simple prescription - original - if applicable
· prescription "prescription" component - if applicable
· prescription "release" component - if applicable,
· copy of the identity bulletin or identity card,
· copy of the pension coupon, not older than two months.
The report and documents are to be submitted to the open-circuit pharmacy within the hospital by mail or in person.
The report is resolved within 30 days from the registration of the request/report at the institution's registry.
